Supplementary Homicide Report
This form can be filled out and submitted in one of two different ways:
Microsoft Excel
Click here to download the Homicide Report form in Excel format. Fill in appropriate boxes. (You will notice that only the boxes colored yellow can be filled in.)
When you have entered all the necessary information, you have the choice of eMailing or "snail-mailing" the report to BCI.
To e-mail it, save the form, then attach it to an e-mail addressed to driquelme@utah.gov.
To mail the report, print the completed form and mail it to:
