This form can be filled out and submitted in one of two different ways:

When you have entered all the necessary information, you have the choice of e-mailing or mailing the report to BCI. To e-mail it, save the form, then attach it to an e-mail addressed to driquelme@utah.gov.
To mail the report, print the completed form and mail it to:
