Obtaining a Copy of Your Driving Record

Reports on a person's driving record (also known as a Motor Vehicle Record or MVR) shall be released by the Utah Driver License Division to requesters who qualify under the permissible uses of the Driver Privacy Protection Act (DPPA).

How to obtain a copy of your driving record (MVR) in person:

To obtain a driving record for someone other than yourself you must qualify under the permissible uses outlined in DPPA. You may be required to obtain a release from the driver of record. If a release is required, please use form DLD60 and have it notarized.

Qualifying high volume requesters are encouraged to contact Utah.gov or contact them by phone at (801) 983-0275 to request a large number of driving records.

 

Mail-in Requests:

If you are living outside the state and need a copy of your driving record, you may print form DLD60, complete the requested information, and have your signature notarized. When we receive this form by mail, along with $6.00 MVR fee, we will mail back a copy of your driving record.

If you need a Certified Driving Record please complete forms DLD60 and DLD266, send along with a $10.75 fee and specify records requested. Certified MVR may take 2 or more weeks to process.
