We accept Cash, Checks, and Money Orders in all of our offices. Credit Cards are accepted in all offices except Moab, Blanding, Nephi, Ephraim, Fillmore, Beaver, Loa, Panguitch, and Kanab.

$7.00 - Each original Commercial driver license endorsement for passengers, hazardous material, double or triple trailer, tandems, or school bus.
$7.00 - Renewal of a Commercial driver license endorsement
$20.00 - Retake of a Commercial driver license written test
$40.00 – Retake of a Commercial driver license skills test
$7.00 - Retake of a Commercial driver license endorsement test
$18.00 - Duplicate Class A, B, or C license

REINSTATEMENT AND ADMINISTRATIVE FEES - EFFECTIVE APRIL 30, 2007

$65.00 - Reinstatement for an alcohol/drug-related offense
$170.00 - Administrative fee for license reinstatement after an alcohol-related offense or an alcohol, drug, or combination of alcohol and any drug-related offense. This administrative fee is in addition to the reinstatement fee.
$30.00 - Administration fee for license reinstatement after confiscation (pick up order).
